Tournament Player Waiver Form (2012 Form)
This is the player waiver form for the North American Pepsi Cup. Please download the form, fill it out and either mail it to a tournament director or hand it in when you check your team in. A Tournament Player Waiver form is required for each rostered player participating in the 2012 North American Pepsi Cup Tournament.
